Name Change Policy
Students and alumni are permitted to request a name change for a previously published dissertation.
Before Requesting a Name Change
Before contacting the Dissertation Office about changing your name in your dissertation, you must complete the Official Name Change form with the University of Chicago University Registrar.
Name Change Request Guidelines
- The name change request must come directly from the author and cannot be submitted by a third party.
- No other content in the dissertation may be altered for any purpose. For example, authors are not permitted to add or remove content, update figures or tables, or make any other changes to the dissertation.
Scope of the Change
- The name change will apply only to the version of the dissertation found in Knowledge@UChicago.
- If the dissertation is also available through ProQuest and you would like to change your name in the ProQuest version, please let Dissertation Office staff know so that we may assist you in accordance with ProQuest's Dissertation/Thesis Correction Policy. Note that ProQuest may charge a fee for updating material.
Steps to Submit a Request
- Contact the Dissertation Office at phd@lib.uchicago.edu with the following:
- Confirmation that you have completed a name change with the University Registrar.
- Your updated registered name.
- A fully corrected PDF of your dissertation reflecting the name change.
(Optional, but strongly encouraged) We also recommend that you complete the following that apply to your situation:
- For researchers, to establish a uniform research identity, sign up for an ORCID iD
- For Alumni, contact UChicago Alumni & Friends to change your name in their directory
- For current employees, change your name in Workday using the “Change My Legal Name” business process
What the Dissertation Office Will Do
- Review the submitted PDF to ensure compliance with the above requirements and constraints.
- Notify you when your request has been received.
- Inform you once the name change has been successfully processed.
